intake of socially undesirable sucrosecontaining foods. These assumptions would never be reliable. Thus, we recommended caution when interpretation of results from this part of our study. The last point raised by Buyken and Liese was that alcohol intake may predict glycemic index of the diet and hence affect the estimates of glycemic index and glycemic load. The reported associations are relative changes in the continuous variable homeostasis model assessment of insulin resistance (HOMA-IR) expressed as HOMA ratios with corresponding 95% CIs. After adjustment for alcohol in the multiple analyses, glycemic index demonstrated an insignificant association with HOMA-IR (HOMA ratio 0.99 [95% CI 0.97–1.02]; P 0.60) and glycemic load demonstrated a significant inverse association with HOMA-IR (HOMA ratio 0.99 [0.98–1.00]; P 0.01).

Eliminating inpatient sliding-scale insulin: a reeducation project with medical house staff.

OBJECTIVE We studied a systematic program to reeducate our medical house officers on how to manage inpatient hyperglycemia without the use of sliding-scale insulin (SSI). RESEARCH DESIGN AND METHODS Patients admitted to the general medical service with diabetes or a blood glucose >140 mg/dl were included. HbA(1c) was measured in all patients, and therapy was modified if the HbA(1c) was >7.0%....

Comparison on Efficacy and Safety of Three Inpatient Insulin Regimens for Management of Non-Critical Patients with Type 2 Diabetes

Background: Hyperglycemia in hospitalized patients is associated with poor clinical outcomes. Scheduled Subcutaneous Insulin therapy has been recommended for better glycemic control. Aims: To compare efficacy and safety of traditional sliding scale insulin (SSI) versus modified 70/30 insulin versus basal plus supplemental scale (SS) insulin regimens for glycemic control of inpatients with diabe...

An inpatient hypoglycemia committee: development, successful implementation, and impact on patient safety.

BACKGROUND Hypoglycemia is a major and preventable cause of morbidity and mortality in the hospital setting. Prevention of hypoglycemia in hospitalized patients relates to the practice climates and prescribing patterns of physicians, the development of safe and effective protocols, and the education of providers and nursing staff on hypoglycemia and its consequences. METHODS Many hospitals us...

A Comparison of Inpatient Cost Per Day in General Surgery Patients with Type 2 Diabetes Treated with Basal-Bolus versus Sliding Scale Insulin Regimens

BACKGROUND The identification of cost-effective glycaemic management strategies is critical to hospitals. Treatment with a basal-bolus insulin (BBI) regimen has been shown to result in better glycaemic control and fewer complications than sliding scale regular insulin (SSI) in general surgery patients with type 2 diabetes mellitus (T2DM), but the effect on costs is unknown. OBJECTIVE We condu...
